To provide initial and ongoing infusion nursing care needs to home care patients complying with applicable laws, regulations, and policies. Triage problems as they arise with home infusion patients. **Characteristic Duties and Responsibilities:** + Fosters continuity of care and facilitates transition of care across the continuum for home infusion patients. + Utilizes the nursing process to perform focused assessments to provide most effective care for home infusion patients. + Demonstrates proficiency with computer skills and obtaining necessary information from EMR. + Independently develops, implements, and evaluates plan of care that recognizes patients’ unique condition and needs. + Incorporates patient/caregiver in planning and implementing care. + Actively engages the patient/caregiver by exhibiting respect and dignity, sharing information in a timely and unbiased manner, and welcoming and encouraging participation to achieve shared decision making. + Accurately assesses patient/caregiver readiness to learn, organizes and executes individualized teaching plan, evaluates patient/caregiver understanding, and modifies approach as necessary. + Provides education reinforcement as needed regarding home infusion to increase adherence to prescribed treatment and improve patient outcomes. + Provides consultation and collaboration for home infusion patients with multiple disciplines including pharmacists, clinic staff, visiting nurse agencies, and Care Management. + Performs telephone triage and troubleshooting for variety of calls regarding home infusion therapy. + Reviews labs, obtains clinical assessments and coordinates care for complex patients on home parenteral nutrition. + Engages in continuous quality improvement by participating in HomeMed’s audit process, patient safety interventions, and incorporating evidence-based nursing practice. + Utilizes own knowledge/skills to improve professional development environment on the unit and participates in committees and research as appropriate. Chemical inventory reference materials are available to all staff in the form of hard copy and on-line Material Safety Data Sheets. Staff may carry out assignments in patient’s homes, hospitals, clinics and PACs offices.
