Our client, healthcare services provider, is seeking a Registered Nurse Case Manager to join their team. As a Registered Nurse Case Manager, you will be part of the home health and hospice support team. The ideal candidate will have compassion, independence, and strong critical thinking skills which will align successfully in the organization.

Job Title: Registered Nurse Case Manager

Location: Marquette, MI

Pay Range: $60,000 - $70,000

Shift: M-TH, Day Shift

What's the Job?

Conduct patient assessments and develop individualized care plans. Provide direct nursing care and educate patients and families on symptom management and end-of-life processes. Coordinate care with interdisciplinary team members and external providers. Maintain accurate documentation and ensure regulatory compliance. Travel to patient homes, participate in on-call rotations, and support patients and families during critical moments.

What's Needed?

Accredited nursing degree with an active RN licensure (BSN or ADN required). Ability to work autonomously in the field, with previous nursing experience preferred. Reliable, efficient, and capable of critical thinking; experience with EMR is a plus. Full-time availability, Monday through Thursday, day shift. Flexible schedule and a recently revised salary and benefits package.
