Registered Nurse Case Manager

As a Home Health Case Manager – Hospital Liaison, you will leverage your clinical expertise and critical thinking skills to design and implement personalized care plans that ensure high-quality, cost-effective outcomes. In this role, you will serve as a vital connection between hospital care and home health services, working collaboratively with patients, families, healthcare providers, insurers, and community resources to facilitate seamless transitions and exceptional care. Your ability to coordinate with multiple stakeholders will help optimize patient outcomes and enhance their overall care experience.

Week : Wednesday through Saturday 8AM-6PM

Week 2: Sunday through Wednesday 8AM-6PM

This position will be based at Intermountain Medical Center

Home Health experience preferred.

Minimum Qualifications

Active RN license in the state of practice.Three years of clinical nursing experience, demonstrating strong patient care expertise.RNs hired or promoted into this role must obtain their BSN within three years of hire or promotion.

Preferred Qualifications

Bachelor’s degree in Nursing (BSN) from an accredited institution (degree verification required).Case Management Certification to support advanced patient care coordination.Experience in case management, utilization review, and/or discharge planning.

Visual & Assessment Skills: Must consistently read and interpret information, labels, monitors, equipment, and supplies to assess and respond to patient needs effectively.Communication & Responsiveness: Frequent interaction with patients, care providers, and visitors requires strong verbal communication skills, as well as the ability to hear and quickly process spoken information, alarms, and urgent concerns—especially in emergency situations.Manual Dexterity & Technology Use: Requires precise manipulation of complex medical equipment with accuracy. Regular computer use and typing are necessary for documenting patient care and accessing vital information.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. 

$44.42 - $65.70
