Are you a compassionate, experienced RN who loves making personal connections and helping patients understand their options at a vulnerable time? As a Hospice Admissions Nurse, you’ll be the face of our care—helping families say “yes” to compassionate end-of-life support when it matters most.
We provide hospice care and palliative care that support patients and their families with dignity, comfort, and love.
**What You'll Do as a Hospice RN Admissions Nurse:**
* Report directly to the Admissions Manager and serve as a key ambassador for our hospice services
* Visit patients and families in their homes, hospitals, or facilities to assess eligibility for hospice care
* Explain hospice philosophy and services clearly, compassionately, and with clinical confidence
* Guide patients and their loved ones through the admissions process, ensuring a smooth and supportive start to care
* Collaborate with referring physicians, case managers, and hospital or facility staff to coordinate transitions of care
* Use the nursing process to assess patient needs, support care planning, and document clinical findings
* Build trust and provide emotional reassurance to patients and families as they make this important decision
* Accurately complete required documentation and ensure compliance with state and federal regulations
* Work closely with the clinical and interdisciplinary team to ensure timely admissions and quality care handoff
**About You**
**Qualifications - What You’ll Bring:**
* Active RN license in the state of employment (or eligible to obtain)
* 1+ year of general nursing experience
* A hospice heart: compassionate, empathetic, and deeply committed to patient-centered care
* Strong assessment, communication, and interpersonal skills
* Comfortable visiting patients in hospitals, skilled nursing facilities, or home settings
* Valid driver’s license, auto insurance, and reliable transportation
* Current CPR certification
**Preferred Experience (Not Required):**
* 2+ years of hospice, home health, healthcare admissions, or medical sales
* Bachelor’s Degree and/or Certified Hospice and Palliative Nurse (CHPN) certification
* Background in case management, care coordination, or healthcare outreach
* Experience with electronic medical records and compliance documentation
