ChristianaCare is currently seeking a full-time Registered Nurse to join our Primary Care Practice on the campus of Wilmington Hospital in Wilmington, DE.
You will be supporting a team of 2 Physicians and an Advanced Practice Clinician who will work closely with and dedicated Pharmacist, Social Worker, and a Community Health Worker working with complex patients in an academic practices setting.

Performs Annual Wellness Visits, Advance Care Planning and Transitional Care Management calls and disease management.Triages walk-in patients and ensures appropriate dispositionTelephone triageAnswers and responds to telephone messages under the direction of the providerAssists providers in examinations or proceduresFunctions as a liaison between patients, physicians and staff to provide accurate communicationAssures adequate coverage within unit when neededSupports the work of LPN and/or MAAssists with staff education and evaluation of clinical competencies for new and existing employees (MA & LPN)Assists with orientation of new personnel to ensure that they are properly trained to support patient and provider.Performs chart audits or has designee assist with chart auditsCase Management and interdisciplinary collaborationChronic disease management under provider directionPerforms nurse appointments for patient education and medication/vaccine administrationIn office medication administration and wound careFull spectrum family medicine practice - meaning prenatal care as wellEducation Requirements:
A graduate of a state approved or National League of Nursing accredited School of Nursing.RN license or eligible for licensure in the state Delaware.Two years’ experience as a RN, physician office experience preferred.BSN preferred; ADN considered with agreement of completion of BSN in 3 years.Special Requirements:
