ChristianaCare, Primary Care at Wilmington Adult Medicine is looking for a full-time Registered Nurse.

ChristianaCare’s Rocco A. Abessinio Family Wilmington Health Center is home to three primary care practices, which includes Wilmington Adult Medicine, as well as other programs and services. Offering wellness and preventive health visits for people of all ages, and medical care for treating illness and injury, the team of clinical and support personnel provide the high-quality health care that every patient within our community deserves, regardless of income or ability to pay. Care teams include nurses, physician residents, attending physicians, and nurse practitioners.
 

Work Schedule:

This full time Registered Nurse position’s days and hours are as follows:

Monday-Friday

8:00AM-4:30PM

Job duties & responsibilities:

Demonstrates skills and knowledge necessary to provide care to patients in the community which include pediatrics, adolescent, adult, geriatric and homeless population. Includes knowledge of growth development

Utilizes experience, knowledge and skills necessary to provide and demonstrate patient centric care delivery as well as identify patient needs

Assures compliance with regulatory and professional standards through regulatory and tracer methodology

Collaborates with interdisciplinary team including Virtual Practice, Community Team, Operational Team

Demonstrates skills and knowledge to meet the needs of underserved populations in community 

Adheres to customer service standards

Triages patient concerns, ensures appropriate disposition and demonstrates follow through on necessary actions

Interacts with patients and provides necessary education using teach back method

Acts as a liaison for patients, families, and health professionals and actively communicates with the patient and team

Assess patient response to interventions

Demonstrates knowledge and skill in direct nursing care

Answers and respond to telephone messages under the direction of the provider

Assists providers in examinations or procedures

Fosters, maintains, and strengthens community relationships that will help care for the patients

Helps coordinate transitions of care.

Responds effectively to patient crisis situations.

Supports work of the Medical Assistant and Community Health Worker

Assists with staff education and evaluation of clinical competencies as necessary

Assist with orientation of new personnel to ensure that they are properly trained to support Community Practices

Randomly shadows clinical support staff to ensure appropriate care is provided

Administration of medications, monitoring of vital signs, and performing routine treatments and procedures prescribed by the provider when needed.

Keeps current with policies and procedures and participates in quality improvement activities.

Enhances the professional growth of self by staying current in clinical practice, participating in teams, projects, councils, committees, Magnet and task forces.

Demonstrates and promotes Christianacare Core Values and Behaviors 

Engages staff in adhering to customer service standards

Performs assigned work safely, adhering to established departmental safety rules and practices; reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.

Performs other related duties as required


Education Requirements:

Graduate of an accredited school of nursing

RN License or eligible for licensure in the state of Delaware

Three to five years of clinical nursing experience in ambulatory practice, medical-surgical, emergency department, public health nursing or home health nursing experience

Prior experience in educating and supervising clinical staff or other informal leadership role 

An equivalent combination of education and experience may be substituted

BSN preferred; ADN considered with agreement of completion of BSN in 3 years

Hourly Pay Range: $38.22 - $61.16

This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
