Responsibilities:
Utilizes the nursing process effectively (assessing the patient and situation, identifying the problem, planning, implementing, and evaluating care through the health care continuum).
• Consistently demonstrates competent clinical skills. Coordinates and completes patient assessment and re-assessment to positively impact on quality outcomes.
• Assesses barriers to learning and functional status of patient. Coordinates, evaluates, and completes patient/family teaching.
• Consistently responds to patients/families in a courteous, positive, and timely manner.
Consistently respects the patients right to privacy and dignity. Consistently respects the patients’ rights to confidentiality according to HIPAA guidelines. • Responsible for in basket management. Assists with pharmacy contact, medication ordering and renewal. Reviews and appropriately disseminates labs based on acuity. Reviews and appropriately responds to patient messages, in collaboration with clinician(s).
• Recognizes emergency situations and acts appropriately. Clarifies physician orders as indicated. Maintains a safe environment for team members and patients/visitors: physical, psychological, and environmental. Utilizes the HRO safety principles.
• Reports all injuries (patient or staff) using event reporting system.
• Demonstrates knowledge of medication and vaccine administration according to policy, carries out physician orders in an accurate and timely manner. Monitors effects of medications and reports adverse effects in a timely manner.
• Demonstrates knowledge of policies and procedures, including but not limited to knowledge and skill of emergency equipment and procedures. Identifies and reports all risk management issues in a timely manner.
• Responsible for phone management of patients. Triages phone calls utilizing critical thinking and assessment skills to evaluate the chief complaint and urgency of the problem. Triages phone calls to determine acuity to assist in scheduling appointments. Provides comprehensive medical record documentation. Documents all communications and maintains patient confidentiality.
• Participates in and/or organizes regular practice huddles, communicates effectively with team members and physicians; actively listens and seeks feedback. Collaborates with team members to solve problems and resolve conflicts. Functions as a resource/mentor and provides support to team members to resolve patient care and team concerns/issues.
• Utilizes standard precautions including, appropriate isolation techniques. Consistently follows the Ambulatory Health Care National Patient Safety Goals.
• Through the model of Shared Governance, collaborates with all members of the health care team with a focus on promoting evidence-based care within a healing culture for the patient population served and their families.
• Demonstrates knowledge of documentation guidelines. Documents patient’s response to the ordered treatment protocol. Reports changes in a patient condition to the clinician immediately. Consistently collects and completes all required documentation. Verifies accuracy of order entry and transcription. Maintains competency in use of all needed information systems. Able to communicate effectively verbally and electronically.
• Pursues and participates in formal and independent learning activities to enhance knowledge, clinical competency, and professional development.
• Monitors quality dashboards for assigned clinicians/practice.
• Performs other duties per operational needs of the assigned clinical practice area and within the scope of RN license.
• Supports the overall strategic goals of the clinical practice care area, AMG, the AHS hospital sites, Atlantic Visiting Nurse, and the system.
