Provides nutritional counseling to clients referred by a provider. Develops educational goals, learning objectives and a plan for educational content and teaching methods. Develops an individualized follow-up plan with each program participant. **Duties and Responsibilities** + Provides group education and individual education in Diabetes Education. + Provides nutritional counseling to clients referred by a provider. + Collaboratively develops educational goals, learning objectives and a plan for educational content and teaching methods with program participants. + Manages blood glucose for patients. + Manages insulin pumps and continuous glucose monitors for patients. + Collects program participant assessment data, using the ADDE7 framework, in a collaborative and ongoing manner. + Collaboratively develops an individualized follow-up plan with each program participant. + Conducts a follow-up assessment upon completion of program services, using outcome measures from the AADE7 Continuum of Outcomes Framework. + Documents assessment data, educational plan, educational services provided and evaluation results in each participant's educational records. + Utilizes a team approach to provide services and collaborates and communicates with team members when needed. + Participates in peer review process to evaluate performance of other professional staff. **Common Expectations:** + In-depth knowledge about current diabetes treatment management. + Ability to develop a collaborative, therapeutic alliance with individuals. + Provides outstanding customer service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation. + Enhances professional growth and development through participation in educational programs, current literature, conferences, and workshops. **Qualifications** **Minimum Education:** + Associates Degree Required + Bachelors Degree Preferred **Work Experience:** + 2 years Relevant experience.
