Medical Facilities of America is recruiting for a Regional Social Work and Discharge Planning Specialist to serve as a resource for Social Work and Discharge Planners at several long-term care centers. This position will support centers in the Southwest Region of North Carolina, including locations in Greensboro, Charlotte, Hickory & Maggie Valley. The Regional Social Work and Discharge Planning Specialist serves as a regional education specialist on the timely and efficient implementation of bed management, discharge planning, internal/external transfers, MDS and care plan accuracy, discharge planning survey management, departmental assessments and documentation requirements, educator on available services, health management, and coping strategies, support for auxiliary and psychiatric needs, enforcing ethical standards and confidentiality, and compliance with company policy and regulatory standards.

The candidate must have strong leadership, documentation, and presentation skills.

Regional travel will be required.
