Regional RN Clinical Coordinator
Must be able to travel locally within region to currently include Newberg, Tualatin, Canby, and Beaverton.

POSITION SUMMARY The Regional RN Care Coordinator ensures that resident care needs are met by coordinating and ensuring assessments are completed and all residents have accurate service plans at assigned locations as requested by regional or community leadership. Provide training and education of staff in all areas of health-related services, policies and procedures as assigned. The Regional RN Care Coordinator supports multiple communities, providing RN services to include on-call support and phone triage, nursing assessments, evaluating and documenting significant changes in condition, performing wound assessments, and conducting on-site routine clinical reviews.


ESSENTIAL DUTIES AND RESPONSIBILITIES Completes or oversees nursing assessments required for significant changes of condition, in accordance with applicable regulations. Assesses health, functional and psychosocial status of residents. Participates in the development of individualized service plans.Supports multiple communities as assigned, providing RN oversight and ensuring timely completion of required nursing evaluations and documentation. Responsibilities include phone triage, on-call support, wound assessments, significant change evaluations, and routine clinical reviews.Provides training of staff in health-related services including but not limited to providing personal care and medication management.Oversees the documentation and communication of resident care and services. Assists the management team to ensure resident records are completed in a timely manner and maintained according to company policies and state regulations.Delegating nursing tasks may be required, as permitted under applicable state nursing laws and practice acts, with appropriate communication maintained with residents, their families, and health care practitioners.Maintains and oversees medication services including staff training, staff oversight, pharmacy coordination, medication room and medication systems.Maintains up-to-date knowledge of all rules, regulations and policies pertaining to resident care. Stays informed of industry and health care trends and standards, applying new knowledge to meet the changing needs of residents. Seeks out additional expertise and advice when needed to assure residents’ needs are met and proper practices followed.Performs other duties as assigned.

QUALIFICATIONS AND EXPERIENCE

Must have current, unencumbered RN license in the state of practice. Experience in wound assessments, coordination of care, weight monitoring and interventions as needed, identifying and troubleshooting shooting for a change of condition required. Experience in geriatric nursing, medication administration, case management, and training and staff development preferred.

Delegation experience in the state assigned preferred.

EDUCATION

Registered Nurse license and Bachelor of Science in Nursing or Associate Degree in Nursing.

PTO:
AL/PCI Salary: Accrue PTO at rate of 0.0615/hour during their first year/16 days.
PCI Hourly: Full-time and Part-Time team members (over 20 hours/week) accrue PTO at rate of 0.0423/hour during their first year.
