Position Purpose

Join Trinity Health PACE as a Medical Director and lead a team dedicated to delivering compassionate, high-quality care to older adults. You’ll oversee clinical outcomes, guide Primary Care Providers, and drive performance improvement across our program.

What You Will DoProvide medical leadership and supervision for participant careOversee clinical quality, safety, and regulatory complianceLead and support the Quality Assurance Performance Improvement (QAPI) programCollaborate with hospitals, SNFs, and community providersMentor and coach clinical staffParticipate in 24/7 on-call rotationEnsure proper documentation and EMR optimizationSupport pharmacy coordination and advance care planning

Minimum QualificationsMD or DO with active state licenseBoard Certified in Internal Medicine or Family Medicine (Geriatrics preferred)1+ year experience with frail or elderly populationsPrior leadership or supervisory experienceValid driver’s license, insurance, and reliable transportationCurrent BLS, CDS Certification, and DEA Registration
