Are you a compassionate Registered Nurse looking for a role where you can make a meaningful impact across multiple communities? We’re seeking a Regional Float Nurse (RN) to provide clinical support to hospice branches throughout central Pennsylvania by stepping in where care is needed most.

ResponsibilitiesAs a Regional Float Nurse, you will travel throughout central Pennsylvania to provide high-quality hospice nursing care and temporary staffing support to local branches. Responsibilities include:
Deliver compassionate, patient-centered hospice nursing careComplete initial and ongoing comprehensive patient and family assessmentsDevelop, implement, and revise individualized plans of careAdminister medications and treatments as ordered by physiciansMonitor patient conditions and communicate changes to physicians and care teamsEducate and support patients and families regarding medications, symptom management, and care plansCollaborate closely with the interdisciplinary hospice team to coordinate careAssist patients with activities of daily living and promote comfort-focused careMaintain accurate and timely clinical documentationServe as Case Manager when assigned and coordinate patient care for designated caseloadsParticipate in interdisciplinary meetings, quality improvement initiatives, and ongoing trainingSupport onboarding and mentoring of newly hired nursesEnsure compliance with all hospice policies, state regulations, and federal requirementsParticipate in on-call rotation as defined by organizational policy Qualifications Graduate of an accredited nursing school with current licensure to practice in state of operation.1 year hospice nursing experience required, 3+ years preferred.Graduate of an accredited nursing school with current licensure to practice in state of operation.Excellent observation, verbal and written communication skills, critical thinking skills, basic math skills; nursing skills per competency checklist.Knowledge of the hospice philosophy of care.Commitment to clinical and documentation excellence.Travel 75-100% About our Line of Business At Grane Hospice Care, our primary ambition is to bring first-class care to the warmest and most serene environment possible: home.
