entry insurance verification and MD verification They build and maintain relationships with referral sources address customer needs and lead a daily intake meetingKey areas of responsibilityPerforms all daily patient referral and intake procedures including insurance verification and financialclinical clearance and ensures patients are admitted in accordance with the intake policy Provides recommendations for process and policy improvements Ensures compliance with all state federal and Joint Commission referralintake regulatory requirementsStays thoroughly informed about the details of contracts the organization has with insurance companies healthcare networks or other partners for Home Health and Hospice services Ensures patient admissions comply with rules requirements and terms in the contractsEstablishes and maintains positive working relationships with current and potential referral sourcesProvides a smooth transition of patients to home care by overseeing patient education care planning and coordination with multiple providers while also negotiating service pricing with insurance Case Managers and other payers within established financial guidelinesStrengthens and monitors community and customer perceptions of Hartford Healthcare at Home as a high quality service provider while collecting and reporting referral data and key customer trendsStrengthens and
