their clients and their clientsrsquo familiesThe Region Clinical Intake Specialist is responsible for managing and coordinating the clinical intake process for both Home Health and Hospice services across multiple branches This role ensures timely accurate and compliant intake procedures using the Homecare Homebase HCHB system serving as a key liaison between referral sources clinical teams and operational leadershipPerforms all clinical aspects of the intake process including communication with patients and families and admitting clinicians clinical data entry accuracy timeliness and Face to Face encounter accuracy and PCPMD verification Ensure patients are admitted timely and in accordance with the intake policy Provides recommendations for process and policy improvements Ensures compliance with all state federal and Joint Commission referralintake regulatory requirementsStays thoroughly informed about the details of clinical quality metrics of the organization has with key Home Care Leadership healthcare networks or other partners for Home Health and Hospice services Ensures patient admissions comply with rules requirements and terms in the contractsEstablishes and maintains positive working relationships with current and potential referral sources and associated clinical leadership teamsProvides a smooth coordination of care of patients to home care by overseeing patient education care planning and coordination with multiple providers case managers and other clinical needs within established payer and financial guidelinesStrengthens and monitors community and customer perceptions of Hartford Healthcare at Home as a high quality service provider while collecting and reporting clinical referral data and key customer trendsWork whereevery
