at every level within the organizationThe Referral Coordinator is responsible for ensuring patients receive timely accurate and well coordinated access to care This position serves as a key liaison among patients providers and external facilities by managing referrals verifying required documentation and coordinating appointments in compliance with organizational and regulatory standardsJob ResponsibilitiesReview and process incoming referrals to ensure completeness accuracy and program eligibilityVerify insurance coverage authorization requirements and referral criteria as applicableCoordinate and schedule patient appointments efficiently to support timely care deliveryCommunicate with patients to explain referral processes appointment details and next stepsCollaborate with physicians surgeons and clinical staff to ensure appropriate patient placement and continuity of careCommunicate with referring providers regarding incomplete or inappropriate referrals or when patients are unable to be reachedMaintain accurate up to date documentation within the electronic medical record EMRCoordinate with external facilities such as dialysis centers and specialty clinics to support seamless care transitionsEnsure compliance with organizational policies HIPAA regulations and quality standardsSkills CompetenciesStrong attention to detail and accuracyAbility to manage multiple referrals simultaneously in a fast paced environmentProfessional and effective communication skills with patients and healthcare providersStrong problem solving follow up and organizational
