and clinical partnerships in a convenient safe and comfortable patient environmentJob SummaryAccountable for using shared life experiences and recovery management techniques to engage motivate educate and mentor clients in achieving individualized goals for successful community living The recovery Support Specialist provides peer support which is a non clinical role This class does not involve treatment assessment or evaluation activities The duties are centered on mutuality and sharing common recovery perspective and language with the client A recovery Support Specialist is expected to act as coach and advocate promoting the program value of autonomy diversity and empowerment with the knowledge and ability to link client to community based supports and other resourcesJob Responsibilities Through collaboration with the client and other service providers assesses needs and develops an Individualized Recovery Plan IRP Utilizing recovery oriented interventionsservices ensures effective progress toward the IRP Goals Provides education and consultation to the Clinical Support Team with respect to client centered approaches Provides instruction and skills training in illness and recovery management to clients and their families Provides engagement in natural support networks and activities relating to community integration Works with Housing Supports Program to assists clients with finding obtaining and retaining safe affordable and decent housing Assists clients in arranging transportation to clinical medical and any other appointments deemed necessary Performs all required duties regarding the maintenance of all program related and administrative datareports statistical records and other data collection activities as specified by Rushford Prepares appropriate documentation to meet DMHAS PR program and other funding requirements Maintains all client records including progress notes service plans and releases Works under the general supervision with significant experience that relates to the population servedRequires current valid Connecticut drivers license Must qualify for Public Service LicenseDemonstrating peer support or mental health experienceHaving personal experience relevant to services provided recoveryBeing able to obtain Peer Support Specialist certification preferably already certified as recovery coach or recovery support specialistBeing able to travel sometimes daily among various Rushford sites be prepared and willing to transport clients as appropriateAs a Hartford HealthCare entity Charlotte Hungerford provides
