Improvement Coordinatorperforms systematic and objective performance improvement activities for both clinical and non clinical areasResponsibilities include but are not limited to the followingAssists in performance improvement activities to monitor the quality of care to meet regulatory standards eg DPH TJC and CMSProvides data abstraction from hospital applications and inputs into internal and external databases to support publicly reported measures and other performance improvement activities Coordinates medical record audits for the state federal andor managed care reviewInterprets regulatory specifications of Core Measures and accurately oversees CMS validation requirementsGenerates analytical reports with statistical quality control tools for specialized and generic performance improvement activitiesCoordinates ongoing screening collection and analysis of informationIdentifies problems that have an impact on patient care and the clinical performance of members of
