Kforce has a client in Denver, CO that is seeking a Project Manager - Value-Based Care Programs to lead the implementation of value-based care delivery models for Medicaid and CHP+ members. This role will oversee the full project lifecycle, coordinate cross-functional stakeholders, and ensure timely resolution of risks and issues. Experience with Medicaid/CHP+ programs is highly preferred. Key Responsibilities: * Program Implementation: Lead the planning, execution, and delivery of care model initiatives for Medicaid and CHP+ populations * Stakeholder Coordination: Facilitate effective communication and collaboration among SMEs, internal teams, and external stakeholders * Risk & Issue Management: Monitor implementation gaps, proactively identify risks, and escalate issues to ensure timely resolution * Governance & Reporting: Develop dashboards, report progress to executive leadership, and manage project governance and financials * Team Leadership: Motivate cross-functional teams and foster alignment with organizational goals
