**Project Manager- Population Health- Ohio**
**Location:** This role requires associates to be in-office **1 - 2** days per week, fostering collaboration and connectivity, while providing flexibility to support productivity and work-life balance. This approach combines structured office engagement with the autonomy of virtual work, promoting a dynamic and adaptable workplace. When reporting to the office, the office assigned is OH-COLUMBUS, 8940 LYRA DR, STE 300. **Alternate Office locations within Ohio may be considered if candidates reside within a commuting distance from an Ohio office.** Travel within the state of Ohio as needed may be required.

**Schedule:** This position will work a first shift from 8:00 am- 5:00 pm EST Monday thru Friday.
The **Project Manager- Population Health** is responsible for designing, implementing and evaluating population health initiatives to increase health outcomes with designated sub-populations through data analysis, community collaboration and member engagement. Responsible for working with business partners to identify and implement small to medium projects that support the population health management strategy. Works with moderate guidance and is responsible for applying project management knowledge, skills, tools and techniques to project deliverables, processes, and systems. Operates within defined parameters using project management methodology.
**Primary duties may include, but are not limited to:**
+ Lead and manage community, provider, and partner relationships to support population health initiatives and drive measurable change.
+ Develop and maintain essential planning documents, MOUs, and contracts to ensure alignment and accountability.
+ Secure project approvals and create detailed communication management plans.
+ Support regulatory and quality reporting for population health teams.
+ Defines project team roles and responsibilities, develops structured work plans, and comprehensive schedules.
+ Oversee deployment planning and facilitate regular project team meetings.
+ Identify, document, and prioritize scope changes; manage the approval process.
+ Maintain and update all project-related documentation throughout the lifecycle.
+ Ensure the right skill sets are aligned to project needs; set and manage expectations with resource managers.
+ Provide performance feedback to team members and managers.
+ Maintain and resolve issues through a structured issue log.
+ Execute communication plans and provide regular project updates to stakeholders.
+ Track ROI, manage project budgets, and monitor time against plan.
+ Develop and oversee testing strategies; conduct project close-out and quality reviews.
+ Obtain necessary approvals, archive documentation, and ensure compliance.
+ Perform additional responsibilities as assigned.
**Minimum Requirements:**
+ Requires a BA/BS in a related field and minimum of 3 years professional project management experience, which at least 2 years are spent leading and directing project tasks; or any combination of education and experience, which would provide an equivalent background.
**Preferred Skills, Capabilities, and Experiences:**
+ Demonstrated success in building strategic partnerships and collaborations that improve population health outcomes preferred.
+ Demonstrated ability to create an effective team environment, build strong relationships, solving problems and issues in a matrix environment and communicating effectively at all levels of an organization is strongly preferred.
+ Project Management Professional (PMP) certification preferred.
+ Experience working in a lead role on one or more projects strongly preferred.
+ Excellent communication skills across all levels of an organization strongly preferred.
+ Experience leading multiple projects simultaneously preferred.
