JOB SUMMARY: The Project Manager (PM) for LOS and Care Management Support is responsible for planning, coordinating, and executing key initiatives that enhance patient throughput, care progression, and post-acute care transitions across the CHS system. This role supports project management efforts across LOS, Care Management, analytics, operations, capacity management, and post-acute partners. The PM is responsible for delivering timely, data-driven insights, coordinating stakeholder engagement, and ensuring alignment on key care management metrics and strategic initiatives. The PM ensures timely insights, effective stakeholder coordination, and consistent alignment on care management goals and metrics. Job Details KEY RESPONSIBILITIES AND DUTIES: Project Management and Coordination Develops timeline, workplans to support throughput, LOS and care management initiatives. Facilitates clear and timely communication between internal and external stakeholders. Supports change initiatives to drive adoption of workflows, tools and modules. Data Management Fulfill ad hoc data requests and analyses to support CHS discussions and decisions. Prepare and distribute LOS reports that are shared across all six entities. Understand dashboards, data definitions and data sources for accurate reporting and stakeholder alignment. Support stakeholders by delivering clear data interpretation and actionable insights to inform their effective decision-making. Meeting Facilitation Coordinate meetings by preparing data, soliciting input from necessary stakeholders in advance, and preparing materials for discussion. Support meetings by agenda planning, material coordination and follow-up. Cross-Functional Collaboration: Partner with care management, analytics, DTS, CH-owned post-acute facilities, and operations teams to implement new initiatives, modules, follow-up on tickets, validate data, and support cross-site initiatives. Risks and Opportunity Escalation: Monitor care progression data to identify risks and escalate key barriers or trends to leadership. Provides follow-up summaries and updates for leadership on progress, challenges and resource needs. POSITION REQUIREMENTS AND QUALIFICATIONS EDUCATION/EXPERIENCE Bachelor's degree in healthcare administration, business, public health, or related field 2–4 years of project management experience, ideally in a hospital or health system setting SKILLS Exceptional organizational skills and attention to detail Proficient in Microsoft Excel, PowerPoint, and collaboration tools; experience with Epic, Tableau, or data dashboards preferred Strong communication skills with the ability to engage clinical and non-clinical stakeholders Comfortable managing multiple priorities and working independently across different entities Ability to analyze complex data, identify trends, and propose process improvements KNOWLEDGE Prior experience with hospital throughput, capacity management/LOS, post-acute coordination, and complex discharges Familiarity with health system performance dashboards and quality improvement initiatives PMP or similar project management certification preferred Salary Range USD $50.25 - USD $69.49 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
