**The role you'll contribute:**
The Program Navigator is responsible for the overall success of their program and provides support for the clinical care coordinator. Support includes ordering and maintaining program supplies, collecting, maintaining and reporting pertinent patient data points, scheduling and preparing the patient education class, coordinating patient appointments within the hospital, answering program phone calls and web inquiries, and navigating patients through the referral process. Actively participate in outstanding customer service and accept responsibility in maintaining relationships that are equally respectful to all.
**The value you'll bring to the team:**
+ Serves as a liaison between the hospital, physician practices and the patient to ensure optimal customer and clinical service and effective communication.
+ Serves as a patient advocate. Sets up patient appointments and services within the hospital, answers program phone calls and web inquiries, and assists guests with travel arrangements such as hotel accommodations and transportation.
+ Navigates all program inquiries. This includes obtaining patient history, obtaining pertinent medical records, and patient demographic and insurance information to refer them to the appropriate physician office and/or testing center.
+ Accesses the patient medical record to obtain program metrics for quality improvement projects. Enters data into appropriate spreadsheets, tracks and monitors trends of patient metrics, creates reports and completes research projects.
+ Coordinates and implements all logistics for the pre-operative education classes and events.
+ Participates in the development of the program by assessing needs of patients and proactively making recommendations to improve our service. Participates in marketing strategy development and activities toward achieving service line goals.
Qualifications
**The expertise and experiences you'll need to succeed** **:**
+ 3 years of experience providing high-level customer service Required
+ 1 year of experience with navigating electronic medical records Required
**PREFERRED:**
+ Associate in healthcare, sociology or other healthcare related field Preferred
+ Additional software continuing education and training Preferred
+ Previous secretarial experience Preferred
+ Previous experience in the clinical aspect of the medical field Preferred
+ Formal customer service training experience Preferred
+ Previous experience working in a hospital or physicians office Preferred
