Minimum Qualifications Five years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR Six years of full-time work experience in any field plus two years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR A bachelor’s degree plus two years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR An advanced degree or a Juris Doctorate plus one year of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services. EXPERIENCE SUBSTITUTION: Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field. Job Duties and Other Information Job Duties: + Implement the Operational Protocol of the MFP Rebalancing Demonstration: Implementation in OCDD program areas (Residential Options Waiver, New Opportunities Waiver, Children's Choice Waiver, public developmental centers, private ICFs/IID, serving adults and children with developmental disabilities). + Provide training, information dissemination, and technical assistance: Regarding transition + initiatives in OCDD waiver programs. The coordinator collaborates with statewide waiver program managers to identify training needs for various stakeholders and to provide technical assistance to support coordinators involved in transition initiatives. + Monitor the status of transition programs: Collect Quality of Life and Quality Management data to identify trends and patterns, conduct root cause analysis, and develop suggestions for addressing findings. Share information with the MFP Rebalancing Demonstration Project Manager. + Participate in policy development and planning: Related to the statewide start-up of the MFP + Rebalancing Demonstration and other initiatives crucial to the transition of adults from ICFs/IID. + Participate in the development and review of My Place (MFP) budgets and reports: Including + annual budget narratives, monthly activity reports, quarterly reports, and surveys. + Participate in development of concise status briefings: Regarding identification, recruitment, pre- move, and post-move activities for executive staff and statewide stakeholder forums, The MFP Rebalancing Demonstration is highlighted in various meetings and reports.
