Program Leader (ACT OMH community-based crisis team)
Program Team Leader Assertive Community Treatment (ACT) Program Full Time: Monday-Friday Glen Cove, NY / travel throughout Nassau County CEC Healthcare (CEC) is a leading not-for-profit located in Nassau County. CEC assists adults diagnosed with mental illness in reaching the highest level of independence with the freedom to ASPIRE HIGHER in their hopes, dreams and accomplishments. We have a great opportunity for a positive, motivated individual to lead our Assertive Community Treatment (ACT) Team, which is a multi-disciplinary, mobile, outpatient OMH licensed program. In this role, you will be responsible for the day-to-day oversight of the ACT program and management of the ACT staff. In this position, you will: + Oversee assistance and training for recipients in activities of daily living, community management, leisure/recreation, socialization, assertion of rights, and communication of basic needs. + Lead team organizational and service planning meetings. + Responsible for direct recipient services as a member of the clinical team. + Provide clinical direction to staff regarding individual cases. + Ensure staff are clear on responsibilities, scheduled activities and daily plans. Regularly conducts individual supervision meetings. + Ensure staff follow the daily schedule and implement Individual Action Plans according to prescribed methods, including documentation of performance in a thorough, accurate manner. We require: + Master’s degree in Social Work or Mental Health Counseling + NYS licensed LCSW or Mental Health Counselor + Minimum of three (3) years related experience with preference given to persons with experience on a multi-disciplinary mobile team. + Clean and valid New York State driver license with ability to travel throughout Nassau county on a regular basis. + Excellent verbal and written communication skills. + Must have the ability to multi-task, establish priorities and manage time, meet deadlines and perform duties under time constraints. + Proficient in computer skills and experience using Microsoft Office. This is a full-time position, Monday-Friday, day hours with on-call responsibilities. CEC offers a competitive salary and comprehensive benefits package. An EOE m/f/d/v.
