ChristianaCare is searching for a Program Coordinator, who will play a crucial role in supporting the operational work within the ChristianaCare Clinical Alliance (CCCA). The coordinator will work closely with the Manager of Population Health Operations and other key leaders to ensure the successful implementation of value-based care contracts. Your main responsibilities will include:

Providing operational support and assisting in the organization, development, and implementation of key initiatives for the CCCA.

Assisting in the development of meeting agendas, presentations, and materials for CCCA Board/Workgroups and Care Standardization projects, including coordinating and scheduling meetings as requested.

Building collaborative relationships with various Essential Services and Clinical Departments to ensure continued engagement and support.

Developing and maintaining strong interpersonal relationships with CCCA Participants and Medicare Beneficiary Representatives, serving as a liaison to resolve issues.

Preparing and distributing operational data reporting and expenditure and utilization reporting to collaborators.

Coordinating status updates with Participants of the Clinically Coordinated Network regarding reports and modules available within the Innovaccer platform.

Supporting huddles formed by the population health team to communicate changes or advances with Innovaccer.

Coordinating the preparation of Provider and Beneficiary Engagement Tools/Resources, including price estimates, tracking status, design, and distribution.

Coordinating Beneficiary Engagement initiatives, such as compiling Beneficiary Newsletter feedback, distributing CCCA materials, and managing email correspondence.

Preparing requisitions, maintaining provider/practice supply inventory and materials, checking receipts for incoming materials, and processing invoices for payment.

Managing the preparation, development, and distribution of annual Participant Status Reports (PSRs) and attestations.

Assisting in the management of policies and waivers, ensuring compliance with the Policy Review Schedule.

Ensuring all CMS Public Reporting Requirements are satisfied and assisting in the general management of compliance with the Medicare Shared Savings Program regulatory requirements.

Coordinating the management of provider and operational agreements via DocuSign, maintaining contract tracker, and follow-up.

Participating actively in CCCA and/or PHSO activities, attending meetings and educational programs as requested.

Preparing CCCA Board Meeting minutes and providing them to the CCCA Board Secretary for review and approval.

The successful candidate possess knowledge of CCCA operations, Medicare Shared Savings Programs, Quality Improvement, and Practice Transformation. They should also have general knowledge of basic quality principals, clinical performance, and interpretation of clinical reports. In addition, the candidate must have strong communication and organization skills.

Qualifications:

Bachelor’s degree in Business Administration or Healthcare Administration is preferred.

3-5 years of experience in healthcare operations.

Familiarity with practice transformation, practice operations, Board support, policy review, and updating is preferred.

An equivalent combination of education and healthcare operations experience may be considered.
