Professional Staff Nurse/Case Manager, Skilled Nursing

The Professional Staff Nurse/Case Manager at UPMC Muncy Place is responsible for delivering comprehensive nursing care and coordinating with interdisciplinary teams, residents, families, and external partners to ensure high-quality outcomes and seamless transitions post-discharge. This role involves managing both short- and long-term residents, supporting care planning, leading Crisis Intervention training, and promoting wellness through chronic condition management and education. The Case Manager collaborates with the Quality Improvement Team to monitor and reduce Emergency Department visits and hospitalizations, identifies educational needs, and provides guidance to frontline staff—all while applying age-appropriate care principles and maintaining a resident-centered approach.
The ideal candidate will have the following: 2 years of RN experience, 5 years working in a Long Term Care facility, and a BSN.
Schedule: Full-time, 40 hours/week (Day Shift, Monday–Friday)
Holiday Rotation: 1-2 holiday shifts per year
Weekend Rotation: Minimum 1 weekend shift every 4 weeks
Sign-On Bonus
+ $15,000 for new graduates (2-year commitment)
+ $20,000 for experienced nurses (2-year commitment)
Key Responsibilities
Resident Care & Case Management
+ Manage care for short-term and long-term residents
+ Collaborate with physicians and care teams to ensure quality care
+ Follow residents with chronic conditions and promote overall wellbeing
+ Assist with care planning and discharge preparation
+ Provide psychosocial support and counseling
+ Monitor and document changes in resident condition or behavior
Quality & Safety
+ Partner with the Quality Improvement team to reduce ED visits and hospitalizations
+ Comply with medication safety and infection control policies
+ Identify opportunities for clinical improvement and innovation
Education & Development
+ Lead Crisis Intervention training
+ Identify and address staff education needs
+ Provide feedback and education to frontline staff
+ Engage in continuous learning and professional development
+ Participate in RN residency programs and seek mentorship

+ Communicate effectively with residents, families, and healthcare teams

Preferred Qualifications
+ Long-Term Care Experience: Preferred
+ Education: Bachelor of Science in Nursing (BSN) preferred
Licensure, Certifications, and Clearances
+ Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification required within 30 days of hire (must meet AHA standards including didactic and skills demonstration components)
+ Registered Nurse (RN) license or Temporary Practice Permit (TPP) required
+ Licensure Requirements:
+ Must hold current licensure in the state where the facility is located
+ If licensed under the Nursing Licensure Compact (NLC), a multistate license from a participating NLC state is acceptable
+ Employees who relocate to the facility’s state must apply for local licensure within 60 days of residency change
