The APP will interface with the Endocrinology consult team and work as a collaborative team member on the Glucose Management Team at the University of Colorado Hospital. Clinical care includes direct patient care, clinical follow-up, lab review and other duties. Knowledge of intensive diabetes management (including multiple daily insulin injections, insulin pump therapy, continuous glucose monitoring, noninsulin therapies, and medical nutrition therapy) will be required in this position. + Performs, orders and/or interprets diagnostic studies within scope of practice and clinical area of practice. + Develops diabetes care plan and reassesses and modifies plan as necessary. + Serves as a clinical consultant to other clinical services in the hospital + Collaborates with all members of the multidisciplinary services and units in the hospital to deliver excellent care + Participates in self-directed continued learning + Educates patients and families regarding management of health condition outside the hospital setting + Maintains CME credits as required for continued licensure.
