Centered Medical Home PCMH work related to this role includes but is not limited to quality reporting utilizing Epic and Excel writing narratives and coordinating with Primary Care offices to gather the necessary evidence for PCMH recognition Outside of annual reporting and transformation this role will support the gap in care work necessary to achieve balanced scorecard goals and payer STAR
