quality improvement and performance initiatives Key responsibilities include quality reporting through Epic and Excel drafting narratives and collaborating with Primary Care offices to compile evidence required for PCMH recognitionThis role requires strong critical thinking skills to identify areas of opportunity lead improvement initiatives and support transformation efforts In addition to annual reporting and PCMH activities the Specialist contributes to closing gaps in care to achieve organizational balanced scorecard goals and improve payer STAR ratingsThis position is in person
