Are you a certified Medical Assistant looking to expand your role beyond the exam room? Join our Population Health team and help drive proactive, preventive care across our network. This unique position supports primary care providers in improving patient outcomes through care coordination, quality gap closure, and health screenings.

What You’ll Do:Support Preventive Care Initiatives: Help identify and close care gaps by reviewing patient charts and preparing for upcoming visits.Coordinate Screenings & Referrals: Use standing orders to place screening orders and manage in-network referrals.Outreach & Engagement: Contact patients to schedule important visits like annual wellness exams, well-child checks, and cancer screenings.Data Management: Transfer quality care data into population health platforms and work with reports to identify high-risk patients.Collaborate Across Teams: Work closely with primary care teams and clinical consultants to support value-based care efforts.Float Across Clinics: Be part of a dynamic schedule—work from our Arcadia office on Mondays, visit clinics Tuesday–Thursday, and enjoy flexible Fridays (clinic or remote depending on needs).What You Bring:Experience: 3+ years in a healthcare coordination role (Medical Assistant, Referral Coordinator, or Utilization Technician).Certification: CMA or diploma from an accredited Medical Assistant program.Tech Savvy: Comfortable with EHR systems and scheduling platforms.Communication Skills: Clear, professional verbal communication and a collaborative mindset.Organizational Strength: Ability to manage multiple tasks, analyze data, and meet goals efficiently.Why This Role Stands Out:No Direct Patient Care: Focus on coordination and data-driven impact.Hybrid Work Environment: A mix of office, clinic, and remote work.Community Impact: Help improve health outcomes for thousands of patients across Maricopa County.

Pay and Benefits

The pay range for this position is $22.00 - $28.00/hr.
