opportunity includemiddot Lead a team of hospice physicians and per diem APRNs caring for patients across Connecticutmiddot Participate in the decision making process in relation to admission to the hospice homecare or inpatient settingmiddot Participating as a member of the Interdisciplinary Team in the development of individualized care plansmiddot Certify re certify when necessary patients eligible for hospice care under the Medicare Hospice Benefitmiddot Consult with attending physicians regarding pain and symptom managementmiddot Coordinate efforts with the attending physician to provide continuity of caremiddot Develop and monitor the medical component of the hospice programmiddot Participate in the development coordination and implementation of hospice services and policiesmiddot Participating in the development of staff and volunteer trainingmiddot Conducts Face to Face visits for hospice admissions and recertification when appropriate as required by CMSmiddot Act as a hospice liaison and advocate with the national state and local physicians healthcare professionals partners and communitymiddot Perform related administrative functions as required andor
