Position Summary
The Primary Care Float Physician provides comprehensive outpatient medical care across primary care clinics within a region of care. This role is designed to offer flexible clinical coverage, ensuring continuity of care during provider absences, high patient volumes, or staffing shortages. The Float Physician adapts quickly to new clinical environments and collaborates with multidisciplinary teams to deliver high-quality, patient-centered care and works collaboratively within a multidisciplinary team.Direct Patient Care
Delivers primary care services, including preventive care, chronic disease management, acute illness evaluation, and health screenings, to a diverse patient populationDiagnoses and treats acute and chronic conditions using evidence-based guidelinesRotates among primary care clinics as assigned, providing coverage for clinicians who are away or when the clinics experience increased demandRefers to specialists when necessaryTimely and accurate documentation, inclusive of direct patient encounters and asynchronous carePopulation Health and Clinical Quality
Supports transitions of care and reduced re-admissions and ED useUtilizes data and risk stratification to identify high risk patients to prioritize outreach and gap closure defined by best evidenceProactively participates in quality improvement initiatives to enhance care delivery and care outcomesCollaborates with a multidisciplinary team including advanced practice providers, RNs, pharmacists and care coordinators/community health workersProvides clinical guidance and supervision to clinical and nonclinical team membersParticipates in team meetings as requestedCitizenship
