Physical Therapist – Home Health

Location: Calumet City, South Holland, Dolton, Riverdale, Harvey, Dixmoor, Phoenix, Posen, Burnham, Hegewisch, Thornton, Lansing, Glenwood, Lynwood


Schedule: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM
Rotating weekends required + 1 summer holiday & 1 winter holiday

Bonus: Up to $10,000 sign-on bonus for eligible external candidates (must be in role at time of payout; not available to current team members)

What You’ll Do

As a Home Health Physical Therapist, you will deliver high-quality, patient-centered care in a home setting, helping patients improve mobility, function, and quality of life. You’ll collaborate with an interdisciplinary team while working independently across your assigned territory.

Key Responsibilities

Evaluate, develop, and implement individualized treatment plans based on patient needs and evidence-based practicesMonitor patient progress and adjust care plans to achieve optimal outcomesProvide clear, timely, and compliant documentation to support care and reimbursementEducate patients and families on treatment plans, self-care, and safety practicesCoordinate care with physicians, nurses, and other healthcare team membersParticipate in case conferences, team meetings, and quality improvement initiativesSupport safe patient handling and promote fall and injury preventionWhat We’re Looking ForStrong clinical judgment and ability to manage a diverse patient population, including complex casesExcellent communication and collaboration skillsAbility to work independently and manage time effectively in a field-based roleCommitment to delivering compassionate, patient-focused careRequirementsLicensed Physical Therapist in Illinois (or eligible for licensure)Minimum 1 year of clinical experience (or strong clinical affiliation experience)Basic computer skills (documentation systems, email, etc.)Valid driver’s license and reliable transportationWork Environment & Physical DemandsTravel required within assigned service territoryWork in patient homes and assisted living settingsAbility to lift up to 35 lbs (with assistance/equipment as needed)Frequent standing, walking, bending, and assisting patients with mobility and transfersWhy Join Us?Autonomy in your daily schedule with strong team supportMeaningful, one-on-one patient careOpportunity to make a direct impact in patients’ homes and communities
