We want you to apply! **Job Summary and Qualifications** **Physical Therapist - PRN/Per Diem** **_Weekend coverage needed; Schedule requires a minimum of 2 weekend days/month._** The Physical Therapist (PT) provides high-quality, patient-centered care by managing the complete process of therapeutic care of an assigned group of patients in coordination with the physician and the patient care team. The Physical Therapist completes patient assessments and participates in developing and implementing individualized care plans focusing on goal achievement, patient engagement, and positive patient experience. The PT provides or directs a majority of the patient’s physical therapy to support patients throughout the continuum of care. We are currently looking for a skilled **Physical Therapist** to join our team on a PRN/per diem/as-needed basis. As a Physical Therapist on our team, you will assess, evaluate, plan, and carry out physical therapy programs to restore maximum reasonable function to our patients. You will work with patients to minimize any disability following disease, injury, or genetically acquired conditions.
