Job Summary and Responsibilities

Are you a passionate and skilled Physical Therapist dedicated to restoring function and empowering patients? Join our collaborative rehabilitation team!

You'll provide hands-on direct patient care, coordinating and modifying treatment to achieve maximal functional independence for patients.

You will:

Comprehensive Assessment & Goal Setting: Interview patients/families for function/history; perform tests/measurements; establish realistic, measurable treatment goals.Expert Treatment & Evaluation: Plan/prepare written treatment programs; administer treatments; evaluate effectiveness and adjust plan.Patient Monitoring & Oversight: Supervise PTAs/Technicians; report adverse reactions/changes to physicians; assist in emergency measures.Empowering Education: Implement/assess patient and caregiver education per Plan of Care.Collaborative Communication: Maintain communications with physicians/healthcare professionals (team conferences, rounds).

Shift: Monday - Friday Days

What You'll Bring:
