Job Summary and Responsibilities

Position Summary:

Incumbent participates as a member of the health care team, working with professional and nonprofessional staff and associated professionals in providing care which provides optimum home care and continuity of care to referred patients, addressing the age of the patients served. As part of a multidisciplinary team, the Senior Physical Therapist must effectively communicate with patients with musculoskeletal disease, evaluate their conditions and develop and implement comprehensive treatment plans to return them to their optimal Level of function. In addition, acts as needed as member of the Hospice interdisciplinary team providing in home assessment evaluation. patient/family/caregiver education and in-service training for both staff and volunteers. Must be organized and consistently follow through with responsibilities in a timely manner, especially in documentation. What differentiates superior performance in this position are people skills: being totally oriented toward the customer while simultaneously solving problems in an independent manner. Incumbent is expected to work independently under broad directives and may have responsibility for specific area(s) of clinical expertise. This job has the potential for exposure to blood and body fluids.  


Job Requirements

Minimum Qualifications:

Graduate of an accredited school of Physical TherapyOne year experience as a Physical TherapistCalifornia Physical Therapist LicenseBLS CPR Certification (Must be through the American Heart Association)California Driver LicenseAuto Insurance
