Responsibilities Perform patient evaluations and develop individualized treatment plans.Regularly reassess treatment plan effectiveness; attend staffing meetings and family conferences.Conduct discharge evaluations, recommend home and community safety modifications, and provide referrals and educational resources.Communicate patient needs and progress to the interdisciplinary team, physicians, patients, and families.Collaborate across disciplines to coordinate care and improve patient outcomes. Qualifications Degree in Physical Therapy from an accredited program.Active Physical Therapist license in practicing state.Certification in Basic Life Support or CPR as per state regulations.At least one year of physical therapy experience is preferred.Knowledge of rehabilitation techniques for complex neurological injuries.Ability to lift 50 pounds; safely move/transfer patients up to 100 pounds.Physical stamina: fine motor skills, visual acuity, extended standing/walking, pushing, pulling, bending, stooping, reaching, kneeling, and positioning.Health screenings or exams may be required per location or state policies.
