Rehab Without Walls® is seeking an experienced Physical Therapy Assistant (PTA) to join our innovative Home & Community Neurorehabilitation Program in Walnut Creek, CA.

This is a **W2 PRN position—not 1099—**offering stability, support, and flexibility while allowing you to deliver meaningful, real-world rehabilitation.

 

Our model brings outpatient-quality care directly to the client, supporting independence and reintegration where life actually happens.

Who We’re Looking For:An experienced PTA seeking a primary therapist assistant role from first treatment through discharge.Interested in a caseload of approximately 15–20 hours per week across 3 days, with consistency and continuity of care.Passionate about functional rehabilitation in real-life environments, such as homes and community settings.Ready to move beyond staged therapy and support clients as they:Safely pick up and manage their own prescriptionsReturn to drivingTransition back to workParticipate in yoga, light hiking, and other meaningful activitiesA clinician who thrives independently while valuing collaboration with OTs, PTs, SLPs, and other specialized clinicians.Why Our W2 PRN Model Matters:

Responsibilities Provides physical therapy services planned, delegated and supervised by the qualified Physical Therapist in accordance with the patient’s Plan of Treatment Confers with Physical Therapist regularly regarding patient’s plan of treatment, status of progress toward goals, questions and concernsAssists with cleaning and maintenance of equipment, treatment and department areasReports any problems with department equipment to the appropriate personnel to ensure that it is maintained in good working orderCommunicates patient’s needs and progress to the Physical Therapist of record and  assists in securing special adaptive equipment for patient to improve function.  Incorporates the use of adaptive equipment to enhance the rehabilitation programMaintains and submits patient treatment and billing documentation in the format and timelines required by the Company, facility, funding source, local, state and federal regulations Qualifications Current unrestricted license as a Physical Therapist Assistant by state in which practicingCurrent Basic Life Support or cardiopulmonary resuscitation certification, in accordance state regulationsA minimum of one year’s work experience as a Physical Therapist Assistant preferredDemonstrates knowledge of rehabilitation techniques related to complex neurological injury Communicates effectively and professionally in verbal and written interactions Demonstrates strong organization skills and attention to detail About our Line of Business Rehab Without Walls, an affiliate of BrightSpring Health Services, is a revolutionary neurorehabilitation program that moves individuals outside the walls of institutional settings and into their natural home and community environment through a continuum of care.
