Astria Health is seeking a Physical Therapist Assistant to join Home Health our team! This is for our Sunnyside office. This is a full time role.
As the Physical Therapist Assistant - Home Health position you will be responsible for providing treatment interventions under the direction of a Physical Therapist based on the patient’s established goals and plan of care. You will:
• Understand and adhere to established Agency policies and procedures
• Provide physician prescribed physical therapy under a plan of care established by the Physical Therapist
• Improve or minimize residual physical disabilities of the patient
• Return the individual to optimum and productive level within the patient’s capabilities
• Participate with all other health care personnel in patient care planning
• Perform all skilled procedure as ordered by physician and according to the plan of care established by the Physical Therapist
• Consult with Physical Therapist regarding change in treatment
• Instruct patients and family members in home programs and activities of daily living
• Participate in in-service programs and presents in-service programs as assigned
• Participate in Quality Assurance Performance Improvement (QAPI) activities as assigned
• Attend patient care conference as scheduled
• Prepare medical records, progress notes and updates care plans for each patient visit in a timely manner as per Agency policy
• Perform services planned, delegated and supervised by the Physical Therapist
• Assist in preparing clinical and progress notes
• Participate in educating the patient and family
• Perform maintenance therapy services under a maintenance program established by a qualified therapist, when acting within the therapy scope of practice defined by applicable state licensure laws
• Perform other duties as assigned
• Collaborate effectively across organizational boundaries.

Graduate of an accredited Physical Therapy Assistant Program Current Washington license Basic Life Support certificationOral and written fluency in the English language
