Job Description
Provide and document examinations and evaluations
Establish functional PT diagnosis, prognoses and plans of care
Formulate treatment goals
Provide physical therapy intervention to patients
Measure and report outcomes
Formulate and carry out discharge plans
Supervise licensed and non-licensed assigned staff or students
Interact and practice in collaboration with other professionals
Individuals must comply with the Texas Physical Therapy Practice Acts and Rules

Skills and Requirements
Baccalaureate, Master, or Doctorate Degree of Physical Therapy.
Current Texas Licensure to practice Physical Therapy.
BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care. Physical Therapy experience in an acute care hospital and/or rehabilitation facility with participation in a multi-disciplinary team.
