*Requirements*
* Bachelor’s degree in Physical Therapy (Master’s or Doctorate preferred)
* Current Connecticut PT license
* Minimum 1 year of physical therapy experience (home health experience preferred)
* Strong interpersonal and communication skills for patient and family education
* Ability to work independently and as part of an interdisciplinary care team
*Position Summary*
As a Home Health Physical Therapist, you will provide expert rehabilitation services directly to patients in their homes, helping them regain independence, improve mobility, and enhance quality of life.
*Responsibilities*
* Conduct comprehensive physical therapy assessments, including functional limitations, mobility status, endurance, and ADL performance
* Develop individualized care plans in collaboration with patients, caregivers, and physicians
* Provide therapeutic interventions to restore function, improve mobility, and manage pain
* Educate patients and families on exercise programs, equipment use, and safety strategies
* Coordinate with other healthcare professionals to ensure continuity of care
* Supervise PTAs and home health aides as needed
* Document care in compliance with agency and regulatory standards
* Participate in quality assurance and performance improvement initiatives
