PHM Analyst

PRIMARY FUNCTION

Deliver data-driven insights to inform and strengthen population health programs, improve patient outcomes, and advance Population Health initiatives. This role collaborates across quality, care management, and clinical teams to support strategic decision-making and optimize program performance through actionable analytics. Works under the supervision of the Enterprise Population Health Program Manager. ESSENTIAL DUTIES AND

RESPONSIBILITIES

This list may not include all of the duties that may be assigned.

1. Monitor and report population health performance metrics across quality, utilization, and cost domains for internal and external stakeholders.

2. Support data reconciliation and performs data validation on analytic output to ensure data accuracy.

3. Track care gap closure performance across payors and support the creation of clinic- or provider-level reports.

4. Assist with data preparation and follow-up tracking for internal and payor meetings, including quality measure trends and action items.

5. Maintain operational dashboards using Excel, PowerBi, or similar tools to provide visibility on key initiatives.

6. Partner with clinical and care management teams to identify areas for improvement and support small-scale initiatives through data analysis.

7. Participate in the implementation and documentation of strategies that support value-based care transformation.

8. Provide general support for reporting and analysis needs across population health and care management programs.

9. Conduct population stratification and segmentation analyses to identify high-risk and rising-risk cohorts for targeted care management interventions.

10. Support predictive modeling and risk adjustment activities in collaboration with clinical and actuarial partners.

11. Monitor compliance with regulatory reporting requirements (e.g., HEDIS, CAHPS, STARS, or other regional equivalents) when applicable.

12. Document and maintain metadata and reporting guides to support analytics transparency and reproducibility.

13. Participate in vendor or payor data exchange evaluations, supporting onboarding and data validation when receiving external datasets.

QUALIFICATIONS

EDUCATION:

· Bachelor's degree in Engineering, Public Health, Health Administration, Economics, Data Science, or related field required.

· Master’s degree preferred but not required.

EXPERIENCE:

· Minimum 2 years of experience in healthcare, analytics, quality or process improvement preferred.

· Experience with population health, managed care, or value-based care initiatives is a plus.

· Experience working with interdisciplinary teams in healthcare or public health settings preferred.

 

KNOWLEDGE, SKILLS AND ABILITIES

· Strong analytical skills and attention to detail are important.

· High Proficiency in Microsoft Excel, Word, and PowerPoint is required.

· Ability to translate data into actionable insights and visual summaries.

· Effective written and verbal communication skills are important.

· Familiarity with data visualization tools (e.g., Tableau, Power BI) preferred.

· Knowledge of coding languages such as SQL and/or Python is highly preferred.

· Ability to manage multiple priorities and meet deadlines in a dynamic environment.

· Collaborative mindset and willingness to work across departments.

· Basic understanding of healthcare quality metrics and value-based care concepts.

· Ability to generate clear data visualizations and summaries for both technical and non-technical audiences.

 

TYPICAL WORKING CONDITIONS

· Non-patient facing.

· May be full time remote/telework.

· Must be based in Mexico.

· Full time remote requires quiet workspace without distractions or interruptions while meeting with internal and external stakeholders.

· Sitting for long periods of time.

· Able to work under emotionally stressed conditions.
