Pharmacy Medication History Technician is responsible for accurately collecting and entering current patient medications into the hospital information system This is utilized by the physicians and providers responsible for admitting and discharging the patient from the hospital Effectively communicates with patients families andor caregivers to perform a patient medication history and construct a pre admission medication listCommunication includes patient interviews and review of internal and external systems with relevant informationRelays any outstanding transition of care issues to the pharmacist andor physicianReconciles and documents the most accurate medication historyDocuments accurately and completely in the medical recordConsults with Pharmacist for any questions before documentationContacts the patientrsquos outside pharmacy or physician to resolve all discrepancies in a timely mannerHandles patientsrsquo own medications to ensure they are secured and documentedPerforms other duties as assigned in the pharmacy for times of minimal activityUtilizes time in most effective and productive manner and completes tasks at the prescribed timeCompletes required continuous training and education including department specific requirementsPerform assigned responsibilities according to department policies procedures standards and regulations within
