A caregiver in this role will provide advanced clinical pharmacy services within Cleveland Clinic’s Ambulatory Heart Failure Clinics. Practicing under a collaborative practice agreement, this pharmacist will independently initiate and adjust heart failure and complementary medication therapies. Key responsibilities include guideline-directed medical therapy (GDMT) initiation and titration, management of pre- and post-cardiac transplant patients, and GLP-1 therapy titration. The pharmacist will collaborate closely with cardiologists, transplant teams, and interdisciplinary clinic staff to optimize patient outcomes and advance the quality of care.

Caregivers in this role will work Monday through Friday from 7:00am to 4:00pm.

A caregiver who excels in this role will: 

Perform pharmacotherapeutic monitoring and provision of pharmaceutical care for patients in the area of service responsibility.Provide general and patient specific drug information to patients, physicians, nurses and other caregivers.Interact and collaborate with other caregivers, especially those in transplant teams.Ensure drug levels are maintained while supporting the transplant teams during transplants.Support clinical pharmacist and technician educational development by participating in departmental staff development programs and provide leadership in the education of staff involved in providing pharmaceutical care to patients.Support clinical pharmacists in providing pharmaceutical care to patients by direct interaction, and formal educational programs.Support and participate in pharmaceutical related research in the area of specialty.Maintain expertise in areas of practice through continual learning, journal review and other educational activities.Educate clinical pharmacists, technicians, pharmacy students, post graduate trainees, allied health professionals, nurses and physicians in pharmacotherapy within the specialty area.Other duties as assigned.

Minimum qualifications for the ideal future caregiver include: 

PharmD or Bachelor’s of Science in Pharmacy from an accredited college of pharmacy as well as completion of a post-graduate residency or fellowship trainingCurrent state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hireAdvanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) Certification through the American Heart Association (AHA) within 1 year of hireCompletion of a post-graduate year 2 residency (PGY2) or fellowship required or completion of a post-graduate year 1 residency (PGY-1) plus minimum of 2 years of experience as a clinical specialist or equivalent role which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring including 50% or more of time spent in the specialty area orMinimum of 4 years of experience as a clinical specialist or of an equivalent role which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring including 50% or more of time spent in the specialty area

Preferred qualifications for the ideal future caregiver include: 

Board certificationExperience in the heart failure or cardiology field

Physical Requirements:

Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time. Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs.Position often includes reaching, stretching, stooping, climbing steps and sitting.Must have normal or corrected vision.Must have ability to clearly communicate verbally by phone or in person
