Pharmacist III - Ephrata Community Hospital - Full-Time - 7on/7off
Schedule Full-Time: 35 Hours/Weekly Nights 9:00 pm - 7:00 am, rotating 7 days on, 7 days off **General Summary** Level III Pharmacists possess the expertise and advanced knowledge to be viewed as a department expert in either the operations or clinical domain. The primary responsibility of the Level III Pharmacist is to facilitate the delivery of safe, effective, and appropriate medication therapy to their assigned patient population(s). In addition, Level III Pharmacists will participate in staff education, professional development programs, and performance improvement initiatives. Level III pharmacists will declare either an operational or clinical focus, dependent on the opportunities available at their facility. The position may involve both distributive and clinical activities with the percentage in each domain determined by the departmental need and employee performance. Applicants for this position should maintain a broad pharmacotherapy knowledge base, exemplify strong communication skills, and employ a collaborative approach to patient care. **Essential Functions:** + Maintains the requirements located in the PCLP Mandatory and Optional Criteria Grids, the Level III Pharmacist. + Provides oversight of safe, accurate, and timely medication order verification including clarification of any medication orders with discrepancies prior to dispensing. + Provides pharmacy management and oversight of: - Integrity and security of medications in all care locations - Investigational drugs - Preparation of injectable chemotherapy - Medication wastage, returns, and recalls - Reconciliation of controlled substance wastage - Compounding of formulations - Automated dispensing cabinets - Medication label components and label quality - Procurement and inventory management - Quality control assessment - Coaches and mentors pharmacy technicians and Level I and Level II pharmacists while staffing in the distributive domain - Pharmacy operations as a pharmacist-in-charge (if applicable) + Provides medication therapy management services in collaboration with other members of the health care team including consultative services pursuant to the department's applicable services which may include: - Pharmacokinetic evaluation, monitoring, and dosing dosing (initiation and daily follow-up) - Total parenteral nutrition (TPN) consults - Anticoagulation initiation and follow-up pursuant to a consult - Renal dose adjustments - Intravenous to oral conversions - Additional clinical opportunities may be available based upon departmental needs, clinical experience, and performance, and may include unit-based pharmacist, antimicrobial stewardship, patient care rounds, or pain management team - Direct patient care as part of interdisciplinary team, if applicable, through development of individualized treatment and monitoring plans for patients - Assists with medication reconciliation during all changes in level of care, including but not limited to, transfers and postoperative care + Provides educational services to patients and healthcare providers regarding medication therapy and/or medication safety concerns. + Participates in the training and education of pharmacy technicians, interns, students, and/or pharmacy or medical residents. + Provides medication event prevention, monitoring, and reporting, with an increased emphasis on patient safety. + Utilizes technology to enhance patient safety by ensuring the proper distribution, monitoring, and use of medications; participates in the assessment of technology. + Participates in quality, safety, and performance initiatives related to medication use at the organizational and departmental levels. Takes a lead role in collaboration on departmental process research activities, including mentoring of other staff through research initiatives. + Adheres to WellSpan Health's medication-use policies and standard work. Develops and modifies polices and standard work related to the use of medications within the health system. Participates in the health system's formulary process, ensuring an appropriate formulary system is in place. + Actively participates on department, facility, or health-system committees. **Common Expectations:** + Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assess and interprets patient age-specific data and provides appropriate age-specific treatment for neonatal, pediatric, and adult patients. Provides patient care for assigned patient age group(s). + Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize process and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required. + Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards. + Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise. + Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation **Qualifications** **Minimum Education:** + Bachelors Degree Pharmacy Bachelor's Degree Required **Work Experience:** + Successful completion of PGY-1 residency with 2 years of hospital experience AND/OR PGY-2 residency. Required or + 5 years Hospital experience. Required or + Internal advancement from the Level II Pharmacist position. Required **Licenses:** + Licensed Registered Pharmacist Upon Hire Required **Courses and Training:** + Board of Pharmaceutical Specialties (BPS) certification.
