Job Description
Join our mission to provide care to Medicaid patients through telehealth and phone consultations. As a pharmacist case manager, you will build rapport and trust with patients, coordinate with their pharmacy and healthcare providers, and manage medication-related activities. You will also assess physical and mental health to develop comprehensive treatment plans.
Responsibilities
Provide telehealth and phone consultations to Medicaid patients.Act as a case manager to support patient needs.Coordinate with patient pharmacies and healthcare providers.Conduct medication reconciliation and ensure medication adherence.Provide medication education and coordinate with other caregivers.Develop treatment plans based on physical and mental health assessments.Re-introduce the program to enrolled patients living in Missouri.Essential Skills
Current Pharmacist License in Missouri.Experience in Medication Therapy Management (MTM).Additional Skills & Qualifications
Experience in ambulatory or insurance settings is a plus.Familiarity with writing SOAP notes.Comfortable using phone, email, and fax for communication.Personable, compassionate, and patient with strong phone skills.Work Environment
Enjoy a fully remote role where you can make your own work schedule, ideally between pharmacy hours of 6 am to 7 pm. You may choose to complete patient consultations over weekends if necessary. Expect 8 hours of patient contact per day with an additional 10-15 minutes of documentation after each call. Pay and Benefits
The pay range for this position is $50.00 - $55.00/hr.
