Description
Patient Assessment & Care Planning: Evaluates patients for home care suitability by reviewing medical conditions, therapy goals, and nutritional needs. Develops and adjusts individualized pharmacy care plans using clinical data and pharmacokinetic principles.
Therapy Monitoring & Optimization: Monitors patient progress, interprets lab values, and recommends therapy modifications to ensure safe and effective treatment.
Medication Preparation & Dispensing: Prepares and dispenses medications with attention to concentration, compatibility, and storage. Ensures compliance with safety standards, DEA regulations, and inventory controls for controlled substances.
Education & Communication: Provides medication counseling to patients and caregivers, and supports staff with drug information and equipment guidance. Ensures continuity of care across settings through clear communication.
Equipment & Incident Management: Oversees medication-related equipment use and maintenance. Participates in quality assurance processes and reports medication or equipment-related incidents.
Professional Development & Compliance: Maintains licensure and professional competency through continuing education. Adheres to all legal, regulatory, and confidentiality standards.
Additional Skills & Qualifications
Graduate of an accredited School of Pharmacy.
Current, active Pharmacy license as granted by the State Board of Pharmacy
One of the following: (a) At least one 1 year of experience in sterile IV admixture procedures (b) knowledge of non-sterile compounding, immunizations and medication therapy management (MTM) (c) completion of an ASHP accredited Pharmacy Residency Program, (d) completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy or (e) equivalent experience as approved by the Vice President (VP) of Pharmacy and/or Sr. Vice President (SVP) of Clinical Services
Pay and Benefits
The pay range for this position is $50.00 - $70.00/hr.
