Job Title: PharmacistJob Description
We are seeking a dedicated and detail-oriented Pharmacist to join a Long Term Care Pharmacy team. This role focuses on data review and IV compounding, including Total Parenteral Nutrition (TPN) preparation. The ideal candidate will bring a strong foundation in pharmacy practice and a passion for delivering high-quality care to long-term care patients.
Responsibilities
+ Perform clinical data review to ensure accuracy, safety, and compliance with regulatory standards.
+ Prepare and compound sterile IV medications, including TPNs, following USP and guidelines.
+ Collaborate with pharmacy technicians and healthcare providers to ensure optimal patient outcomes.
+ Maintain accurate documentation and participate in quality assurance initiatives.
+ Support weekend operations on a rotating basis.
Essential Skills
+ PharmD from an accredited college of pharmacy.
+ Active Minnesota Pharmacist License in good standing.
+ Minimum of 2 years of pharmacy experience, preferably in long-term care or hospital settings.
+ Direct experience with IV compounding and TPN preparation is highly preferred.
+ Strong attention to detail, organizational skills, and ability to work independently on 2nd shift.
Additional Skills & Qualifications
+ Experience with prior authorization and order verification processes.
+ Strong compounding and prescription review skills.
Work Environment
This position will work 2nd shift Monday through Friday, from 3 PM to 11:30 PM. It is a contract-to-hire opportunity offering a unique schedule that provides every other week off.
Pay and Benefits
The pay range for this position is $62.00 - $65.00/hr.
