The Hospice Clinical Pharmacist will support the operations of OnePoint Patient Care’s clinicalteam in our East Region and will report to the Vice President of Clinical Services.
Responsibilities Ensure compliance with Federal, State and local laws and regulations, and OPPC/Clientcontract terms pertaining to such interventionsPerform and document Medicare Condition of Participation (CoP)-compliant medicationreviews for new hospice patient admissionsAttend hospice interdisciplinary team / group (IDT / IDG) meetings to discuss patients andprovide insights and guidance regarding the use of medications to palliate commonsymptoms experienced by hospice patientsParticipate in and provide clinical support to hospices by way of participation in Pharmacyand Therapeutic (P&T) committeesConduct and present comprehensive drug utilization evaluations which analyze medicationutilization data for compliance with Medicare CoPs and opportunities to reduce avoidablemedication costs via de-prescribing and therapeutic substitutionAuthorship / presentation of core OPPC clinical offerings, including webinars, companynewsletter, and symptom guide bookProvide evidence-based medication therapy advice during consults with hospice cliniciansUpdate hospice formularies with consideration of hospice-specific financial targets,medication pricing methods, and utilization patternsProvide clinical education and training to hospice clinicians and OPPC traineesProvide preceptorship to Advanced Pharmacy Practice Experience (APPE) pharmacystudentsMaintain appropriate records of consultations and meetingsTravel as necessary to provide local, regional, and national support to our clients, companypharmacies and sales and marketing efforts Qualifications PharmD2+ years hospice or palliative care pharmacy experienceStrong familiarity with Microsoft Office suiteAbility to multitask and prioritize tasks when faced with competing prioritiesExtensive knowledge of drug products, pharmacokinetics, symptom and disease-statemanagement, and federal and state hospice rules and regulationsAbility to communicate clearly in oral and written form to patients, caregivers, clinicians andadministratorsComfort with delivery of presentations to a variety of audience types and sizes in a variety ofvenuesAbility to establish and maintain working relationships with various members of health careteamsAbility to perform data analysis and present drug utilization evaluations and other hospicepharmacy analytics as necessaryAbility to conduct literature searches to identify, review, relate and summarize journal articlesfor use in clinical offeringsPreferred Qualifications:Board of Pharmacy Specialties (BPS) board certification(s)Completion of PGY-1 or PGY-2 residency program focused on hospice or palliative care About our Line of Business OnePoint Patient Care, an affiliate of PharMerica, is the nation’s leading independent provider of community-based hospice pharmacy and pharmacy benefits management (PBM) services – offering hospices nationwide flexible and adaptable solutions for their hospice pharmacy needs.
