**Job Purpose:**
Provide a concierge type service to our patients as they enter our health system for perinatal care via personal calls to assist in patient registration, doing tours of the perinatal units and assisting in patient education classes as needed. Responsible for facilitating the Women’s and Children’s Services bereavement program: including education of the staff and follow up with the patients and the family. Support families, staff and palliative care team in dealing with process involved in withdrawal of life support.

**Education:**
Graduate from an accredited school of nursing, BSN required
**Licenses and/or Certifications (Required and Preferred):**
State of TX RN, Specialty Certification preferred
Current CPR certification required
Current Resolve through Sharing Training
**Related Work Experience and Other Skills:**
Ability to remain flexible and adaptable to change. Able to develop and maintain good working relationships with all levels of staff, patients and family members Able to organize work and work area with time constraints in a fast paced environment Able to professionally and effectively communicate (orally and in writing). Demonstrates accountability for own actions. Demonstrates understanding of MHD guidelines and reference, Infection Control guidelines, and MHS safety guidelines. Excellent oral and written communication skills Maintains confidentiality of patient and business information Work Experience: 2 years experience as a women services nurse with documented Resolve through Sharing training course
**Job Roles:** **_Perinatal Care Coordinator_**
**Duty 1: Pre-Admission Registration / Education**
+ Touch base with families near time of delivery
+ Promote hospital tours and online classes
+ Mail postcard after delivery
+ Compile data on attendance
+ Assist with live classes as able
+ Schedule tours with families
**Duty 2: Bereavement Care**
+ Provide Education to MDMC staff (WCS and ED) on bereavement support
+ Provide education to referral facilities and sister hospitals on bereavement care
+ Follow all perinatal losses for at least one year
+ Maintain bereavement supplies
+ Assist staff with bereavement care during regular hours and as able during off times
+ Meet with all inpatient families experiencing perinatal loss
+ Maintain data on perinatal losses and bereavement support
+ Provide for annual bereavement support activity
**Duty 3: Palliative Care**
+ Partner with Pastoral Care and Palliative care to create neonatal palliative care team.
+ Provide resources for families who chose to implement neonatal palliative care.
+ Collaborate with Maternal/Neonatal Outreach Coordinator to meet with physician groups/MFM to promote early intervention for families with prenatally diagnosed fetal anomalies incompatible with life.
+ Work with NICU staff to support family and staff during process of withdrawal of life support or development of palliative care plan.
+ Follow families implementing hospice care post-discharge to provide ongoing emotional support.
