**Per Diem RN HOSPICE-Care Manager for Nursing Homes in the Capital Region. Come experience the rewards of working in end-of-life care!**
The Community Hospice has a wonderful opportunity for a per diem RN to work as part of our team caring for patients and families in local nursing homes.
Availability needed for this per diem position would be **during the week (Mon-Fri).** Hours generally needed are 8a-4:30p. No weekends needed at this time.
**Position Highlights:**
+ National pre-taxed mileage reimbursement
+ Meetings and charting conducted via supplied lap top
+ Comprehensive orientation provided
**What you will do:**
+ As an RN you will visit hospice patients in the nursing home setting to respond to symptom management needs and coordinate with our team to provide added support to residents, families, and facility staff.
+ Per diem needs would be days would be during the week (Monday to Friday) and hours would be 8 am to 4:30 pm.
**What you will need:**
+ A current license to practice as a Registered Nurse in the State of New York
+ Associates Degree in Nursing, BSN preferred
+ The RN must be able to communicate effectively, verbally and written
+ 1 year experience as an RN required, preferably in long term care.
+ Previous Hospice experience preferred but not required.
+ Valid Driver's License and Reliable vehicle
+ High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers

**Special Per Diem Rates based on monthly hours commitment**
_Experienced Pay Range for non-per diem: $35.00-$47.75_
_Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location._
